  • Can I add family members? Yes! You can enroll your spouse or dependents. Each member will require their own designated plan based on their age and dental health needs.
  • When do my benefits start? Immediately! There are no waiting periods. As soon as you sign up and pay your activation fee, you are eligible to use your preventative benefits and treatment discounts.
  • Can I use this with my current dental insurance? No. This membership plan cannot be combined with traditional dental insurance, medical insurance, CareCredit promotional periods, or any other discount plans.

  • NOT DENTAL INSURANCE: This membership club is a direct payment arrangement provided by Gagandeep Kandola DMD, Inc. (DBA Synergy Dental Arts) for services rendered. It is NOT dental insurance, nor is it a health care service plan regulated by the California Department of Managed Health Care (DMHC) or the California Department of Insurance.
  • For Uninsured Patients Only: Memberships are provided exclusively to uninsured patients of Synergy Dental Arts.
  • Not Pre-Payment: This agreement shall not be considered pre-payment for future services. Members are electing to make regular subscription payments for preventive care and access to a discounted fee schedule.
  • Time of Payment: Payment for any cosmetic or restorative treatment is due in full at the time of service. Membership dues must be current in order to receive services or discounts.
  • Non-Combinable: Wellness Club discounts may not be combined with any other offers, discounts, dental insurance, medical insurance, or worker's compensation.
  • Lapse in Coverage: A lapse of 29 days in dues will require a reactivation fee of 6 months' dues.
  • Refunds & Scheduling: It is solely the patient's responsibility to schedule and keep their appointments. No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.
  • Changes in Fees: Monthly dues and fees for dental services may change at any time with proper notice.
  • Specialist Referrals: The membership discount applies only to services rendered at Synergy Dental Arts. It does not apply to treatments performed by outside specialists to whom you may be referred.
